Services for J.B. Wallace M.D., 77 Ada are 1 p.m. Tuesday at St. Luke’s Episcopal Church, Rev. John Norvell will officiate.

Dr. Wallace died Thursday, Nov. 10, 2011, at an Oklahoma City hospital. He was born June 18, 1934, in Hardtner, Kansas, to Neil and Leota Kennedy Wallace. He received a Bachelor of Science degree in Chemistry from Northwestern State University, in Alva, and then graduated from O.U. School of Medicine in 1964. Dr. Wallace practiced obstetrics and gynecology in Ada for over 40 years. He delivered his last baby in 2008. He had served in the National Guard and was a long-time member of St. Luke’s Episcopal Church.

He and Phyllis Ann Allen were married Oct. 12, 1989, in Ada.

Survivors include his wife, Phyllis, of the home; three daughters, Cathy Mason and husband Brent, Tacoma, WA, Jamie Wallace, and Amy Ballagh and husband Garet, Ada; two sons, Bryan Allen, Stillwater, and Brent Allen and wife Melanie, Adair; one brother, Gary Wallace and wife Jo, Lawrence, KS; two grandchildren, Logan Biddy and Gavin Ballagh.

He was preceded in death by his parents.

The family says those who wish may make memorials to Oklahoma Medical Research Foundation, 825 NE 13th, Oklahoma City, OK 73104.
