Vernon Lee Forman

???? ~ October 29, 2008

Vernon Lee Forman, the son of Leland W. Forman and Hazel Lee Osborn-Forman passed away in Tulsa, Oklahoma on October 29, 2008. He was born in Ponca City, Oklahoma and raised in Wichita, Kansas. He spent his life living in Arizona until 1999 when he moved to Stilwell, Oklahoma to be with his youngest daughter.

He is preceded in death by his parents; Leland and Hazel Forman and his sister Betty Forman-Lybyer.

His surviving family includes two sons; Thomas Forman of Florida, Russell Forman of Oregon, three daughters; Charlotte Forman and Susan Forman of Arizona and his youngest, Santanna Forman-Martin of Oklahoma. His grandchildren include; Dustin and Joseph Shargani of Arizona, Thomas Forman "Lil' Tom" of Florida, Amber and Danielle Forman of Oregon, Zahary Forman of Arizona and Chance Lee Martin of Oklahoma. He had a large number of Great grandchildren and many friends surviving him.

Private services were held Friday, November 7, 2008 at Honey Hill Church in Stilwell, Oklahoma. Vernon will be very missed and forever loved.
