Jacqueline Jean Ballard was born May 23, 1950 in Oklahoma City to Billy Gene Ballard and Jacqueline (Moore) Bassham. She passed away August 11, 2019 in Oklahoma City at the age of 69.
The family will host a private memorial service in her honor. Arrangements are under the direction of Lehman Funeral Home of Wellston.
