Dyzen Wayne Prouty was born on April 5, 2011 at St. Mary’s Medical Center in Enid, OK to parents Kayla Pembrook and Jason Prouty and passed away the same day.
He leaves behind his parents; maternal grandparents Ron Goldston, Jr. and wife Terri of Longdale, OK and Susan (Pembrook) Tarver and husband Joe of Fairview, OK; paternal grandparents Cheryl Prouty and Darrell of Fairview, OK and Steven Ward of Bucklin, KS; great-grandparents Mary Young of Canton, OK, Mary Ann Meehan of Longdale, OK and Harley Pembrook of Fairview, OK; two aunts, Tasha Pembrook of Fairview, OK and Stephanie Prouty of Fairview, OK; four uncles Steven Prouty of Canton, OK, Anthony Tarver of Fairview, OK, Zachary Huber and Ron Goldston, III of Longdale, OK; Numerous great aunts, uncles and cousins.
He was preceded in death by his great grandmas Maxine and Ada; great-grandpas Jack, Sydney and Ron Sr.; great-great grandmas Norma Jean and Martha; great uncles Chet, Billy and George and great aunt Cherise.
Graveside services will be held at 11:00 a.m., Saturday, April 9, 2011 at Fairview Cemetery with Father Prabhakar Kalivela officiating.
