A memorial service for Gavin Samuel Zelnicek, 12, will be 9 a.m. Thursday at Hennessey High School Auditorium.
Unknown DateMass of Christian Burial will be 1 p.m. at St. Joseph's Catholic Church. The Rev. Roberto Quant will officiate.
Burial will be in Calvary Cemetery. Arrangements are by Cordry-Gritz Funeral Home, Hennessey.
He was born Aug. 14, 1995, in Alamosa, Colo., to Eugene T. and Debbie Price Zelnicek and died Sunday, Sept. 16, 2007, at OU Children's Medical Center, Oklahoma City. He was a member of St. Joseph's Catholic Church, Boy Scout Troop 134 and played football, basketball and track at Hennessey Middle School.
Surviving are his parents, Eugene T. Jr. and Debbie Zelnicek of Hennessey; three brothers, Grant Zelnicek and Gage Zelnicek, both of Hennessey, and Loggan Johnson of Cashion; one sister, Meggan McReynolds of Stillwater; grandparents, Eugene T. Sr. and Paula Zelnicek of Hennessey, Sharon and Gary Roles of El Reno and Lynn and Judy Jenson of Enid.
