Monz Warren Green, male, White, never married, was born September 5, 1931, in Oklahoma.

Father: Charley H. Green, born Missouri.

Mother: Ona White, born Missouri.

He lived 12 years in Casa Grande, Pinal County, Arizona. Residence: 924 North Coolidge Avenue, Casa Grande.

Occupation: truck driver, shoes.

At the time of his death in January, 1962, he was age 31 years.

Cause of Death: gunshot wound through frontal area of head. Inquest Pending.

Place of Death: five miles South of Casa Grande.

Informant for Certificate: C. H. Green, Casa Grande, Arizona.

Removal of remains January 13, 1962 to Bridgeport Cemetery, Bridgeport, Oklahoma.
