In 1931 Jennie Nelms Falling and Calvin Ellis Falling issued a warranty deed to the Cherokee Nation of Oklahoma for a two acre site for the cemetery. This became Cherokee Cemetery Patient No. 304 Rogers County and is one of the oldest Indian cemeteries presently maintained in Oklahoma. It was previously known as the Nelms Family Cemetery.

In 1926 Jennie Nelms Falling and Calvin Ellis Falling donated one acre of land adjacent to the cemetery to the general council of the Assemblies of God for a church site. The church was built by the Falling Nelms Family and community volunteers in 1941. The land for the church and cemetery was received from the Cherokee Nation of Oklahoma by Ellen Nelms as an allotment from the Dawes Indian Rolls of 1906 and inherited by Jennie Nelms Falling from her mother Ibbie Nelms. All were Full Blood Cherokees whose ancestors are on the Emigration Rolls of 1835 and the Drennen Rolls (Trail of Tears) of 1852. The Falling Family are also on the Dawes and Drennen Rolls.
